Job Description

Summary of Function: 
The Student Care Coordinator predominately serves the Arizona Western College community by supporting the organization and planning of exemplary Health and Well-being programs and initiatives. Reporting directly to the Manager of Student Health and Wellness, The Student Care Coordinator works closely with campus partners to promote and facilitate health and wellness education and awareness themes and campaigns for healthy and high-risk behaviors for students across the district. Overseeing a student of concern caseload, the Student Care Coordinator supports the education and guidance of students, supporting them with college and community resources and referrals to eliminate barriers and facilitate student success. Serving as a member of the Behavioral Intervention Team (BIT), the Student Care Coordinator additionally supports the Manager of Student Health and Wellness with BIT meetings, manuals, and planning efforts to ensure that the BIT and Health and Wellness department goals are consistent with health and well-being advancement with programming and services.

Typical Duties: 

  • Serves Students. Serve students. Manages Student care cases to support college processes and community referrals. Meets with students to establish rapport, administer behavioral intervention and risk assessments, provide referrals, develop a care plan, and conduct follow-ups and reassessments as needed. Uses exceptional listening skills to understand student needs and honor their culture and/or diversity. Maintains accurate/detailed case information in the database and knowledge of community resources. Collaborates with the Behavior Intervention Team for support and referral strategies. Conducts student outreach through informational tabling events. Follows standards set by NABITA in Risk Rubric and case management processes.
  • Train students, faculty, and staff on the student of concern referral process and promote available resources. Works closely with the Manager of Student Health and Wellness to organize and promote pieces of training each semester on topics such as Mental Health First Aid, Suicide Awareness and Prevention, Vet Ally, Bystander Intervention, and Stress Management. Encourages and supports education surrounding monthly health and wellness and violence intervention and prevention awareness themes.
  • Works closely with the wraparound specialist and manager to support Cubby processes. Coordinates and maintains donation partnerships. Supports grant opportunities for basic needs and wellness activities.
  • Oversees the planning, implementation, and evaluation of theory-based and evidence-informed well-being practices, programs, and services. coordinates, facilitates, and assesses educational workshops and presentations for students on a variety of topics, including physical health, mental health, self-care, and services available to students via Health and Wellness, including but not limited to BetterMynd, and Title IX support. Partners with department staff to execute outreach events. Partners with other well-being areas on programming and education initiatives, including, but not limited to Residence Life, CAMP, KEYs, Accessibility (OSA), Athletics, and Student Events and Activities.
  • Advise the student lead club or association that promotes health and wellness and violence Intervention and prevention from a peer standpoint. Establish leadership opportunities for student participants through training and participation in the promotion and presentation of awareness campaigns and community service.
  • Plans and evaluates a comprehensive well-being program. Maintains programming financial request and event/education record-keeping. Writes reports, develops surveys to be distributed to the student community, and responds to routine correspondence promptly. Supports the Manager of Student Health and Wellness by developing and accessing health and well-being education results and outreach and Student Learning Outcomes (SLO) for program reviews and assessments.
  • Supports and participates in strategic initiatives and performs other duties as assigned.
