Key Responsibilities
Certified Nursing Assistant:
- Responsible for attending to the daily living and safety needs of an assigned group of patients.
- Assist registered nursing personnel in the care of patients by performing various patient care activities and related non-professional services.
- Collaborates and coordinates with other departments to provide timely effective care consistent with individual’s needs, choices and preferences.
- Obtains and records vital signs, heights and weights per policy.
- Meets patient’s personal hygiene and basic needs in a timely manner.
- Ensures that the patient is comfortable, necessary supplies are available and in reach.
- Keeps patient environment safe and tidy.
- Provides patient specific care as appropriate to age of patient.
- Demonstrates effective communication skills with children, adolescents, adults and geriatric patients to include use of appropriate therapies such as play, written, and picture board.
- Reports changes in patient’s condition, out of normal values, patient/family concerns or complaints to nurse, charge nurse and/or supervisor.
- Applies AIDET as well as other evidence-based behaviors.
- Participates in developmental and organizational initiatives for process improvement and quality assurance as requested.
- Follows appropriate infection control procedures.
- Performs skin care interventions and treatments, as permitted by state regulation.
- Maintains patient and staff confidentiality.
- Assists in the orientation of new staff as requested.
Unit Specific Responsibilities:
Med/Surg, Peds, PCU, ICU, Observation Unit, and ER:
- Under the direction of a registered nurse (RN), provide continuous visual and proximal contact with the patient(s), including but not limited to in-room observation, provision of basic comfort services, maintaining a safe environment, accompanying patient during transportation for tests/procedures, and documentation on appropriate forms
- Maintains educational requirements and training requirements to fulfill the role and responsibilities of a
- Unit Coordinator as outlined in the Unit Coordinator job description.
- Regularly updates patient communication boards.
- Performs hourly rounds with patients.
- Performs bedside shift report.
- Assures patient belongings are accounted for on admission and transfers and returned at discharge.
- Performs designated admitting tasks.
- Records accurate intake and output for each patient.
- CNA may be required to provide backup support a Camera Tech in specific units:
- Demonstrates competence with operating video monitor equipment by managing the video monitor (including audio component), display of rooms, adding and deleting beds monitored; is aware of actions for video monitor Downtime.
- Receives report on video patient assignment from nurse. Stays at the monitor at all times, reports changes in patient’s behavior/status to nurse.
- Documents and signs “Record and Video Monitoring form” (verbal reminders, staff interventions, # of fall alerts.
- Identifies the behaviors that may precipitate a fall; is aware of the proper steps to take if patient is not seen on the video monitor.
Home Health and Hospice:
- All documentation is completed accurately and timely (no later than 8am the next day).
- Productivity to average no less than 5-6 patients per day.
Hospice House:
- Performs hourly rounds with patients.
- Performs bedside shift report.
- Assures patient belongings are accounted for on admission and transfers and returned at discharge.
- Performs designated admitting tasks.
- Records accurate intake and output for each patient.
