Job Description
Position Purpose: As the Physical Therapist in Home Health, you will be responsible for providing a full range of Physical Therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment.
Key Responsibilities:
- Provides a full range of Physical Therapy services including assessment, treatment planning, and therapeutic interventions in an interdisciplinary environment consistent with the position’s qualifications, professional practices, and ethical standards
- Uses AIDET, perform hand hygiene, assists with positioning patient for comfort and pain management, and performs environment check prior to leaving patient’s home and maintains a clean and organized environment
- Supervises Physical Therapy Assistants
- Provides patient and/or family education to help achieve goals
- Documents course of patient care including progress and continuing need for treatment
- Participates in multidisciplinary meetings ensuring patient continuity of care
